安全守則與使用規則

  • 第一次來到原岩攀岩館,須仔細閱讀各分店之安全守則與規定內容,並簽署「原岩攀岩館安全守則與個人責任聲明」。入場不攀爬者亦須進行簽署。

  • 初次來館者須接受館方人員教授及確認「抱石安全墜落」等安全必要事項。若無法按照指導完成必要的安全事項練習,為了您的安全,館方有權拒絕您繼續攀爬。
     

未成年者入場

  • 未成年者(未滿18歲)須由本人及法定代理人共同簽署【原岩攀岩館安全守則與個人責任聲明】

  • 18歲以下之未成年者須由成年人監護陪同 ( 冬夏令營等課程學員於上課時間內不在此限 ),方可入場攀爬。陪同之監護人應負連帶責任。

  • 陪同12歲以下兒童入場時,監護人應隨時留意兒童的狀況,並親自管束、確保兒童之安全。除兒童區以外,請勿讓孩子於場館其他區域奔跑及嬉戲。
     

場館規定

  • 禁止攜帶危險物品入場。場內禁止吸菸或飲酒,並禁止任何明火事項。

  • 館內禁止打赤膊或赤腳攀登。上牆時請務必穿著攀岩鞋。

  • 所有食物及飲料(包括運動飲料)只可在飲食區內食用。飲食區外可飲水,但請勿於軟墊上飲用。

  • 所有垃圾請自行攜出,館內無提供垃圾桶。

  • 若場館可讓寵物入場,寵物須安置於場館規定之位置。若寵物有吠叫、驚嚇或影響館內客人等行為,館方有權請飼主將寵物帶離場館。

  • 蓄意破壞、盜取岩館設備者,除依法嚴辦外,須照價賠償。

  • 欲於館內進行任何商業相關行為,包含拍攝商業影片或照片,請事先詢問並取得場館同意。

  • 館方得因定線、教學需求或活動等事由暫停開放部分區域或路線,請配合勿攀爬該區域。

  • 請勿攜帶貴重物品來館,並請自行妥善保管重要財物。館方不為任何財物損失負責。

  • 本場地已辦理公共意外責任險,保險額度依照政府法令規定投保。入場費用不含旅平險與個人意外保險,如有需要請自行投保。
     

安全守則

  • 開始攀登前請衡量自身狀況,並請根據自己的能力選擇攀爬路線及難度,避免超過身體負荷。並請確實熱身及收操,避免運動傷害。

  • 上牆攀爬前,請先確認您的路線不會與已在牆上攀爬的其他人重疊。若發現您與他人的距離太近或路線可能重疊,為了雙方安全,請中止攀爬、回到地面並退出軟墊區。

  • 請尊重先上牆的人之岩牆優先使用權。當牆上有人攀爬時,請勿以任何形式逗留在可能影響對方的區域(包括牆面與軟墊)。

  • 通過攀爬區域時,請先確認不會影響攀爬者,並請盡速通過。

  • 位於攀爬區域時,請隨時注意上方及周圍。抱石區域嚴禁坐在軟墊上,並建議與牆面保持至少2公尺距離,以確保自身及攀爬者安全。

  • 抱石區皆設有軟墊,請以正確安全墜落姿勢從適當高度跳下。跳下前請先確認落地處是否有他人或物品,也請注意軟墊邊緣安全墜落範圍,以避免墜落於軟墊邊緣而發生危險。

  • 離開牆面後,請盡速離開軟墊,禁止在軟墊上逗留

  • 禁止於攀爬區地面或軟墊上放置個人物品

  • 若發生岩塊鬆動、軟墊破損或意外事故,請立即通知現場工作人員。

  • 規定內容若有未盡事宜,岩館工作人員得依現場狀況做適當處置,並修改相關辦法。

  • 若觸犯安全守則規定且經工作人員勸阻不聽者,最嚴重將取消其會員資格,並禁止再次進入原岩攀岩館。
     

個人責任聲明

  1. 本人知悉攀岩活動(包含抱石及上攀)為一項具有受傷甚至致命風險之運動,簽署此責任聲明代表本人已了解安全攀登、安全墜落方式等與攀岩相關之安全規定。本人除願意遵守所有相關規定外,亦理解攀岩運動可能造成的風險、危機、人身傷害、死亡、或財物損失,並願為自身安全及個人行為負有相關之責任。

  2. 本人了解攀岩活動是一項消耗體力的運動,本人已評估自身健康狀況適合進行攀岩,且無隱瞞任何既有之健康及心理問題。

  3. 本人同意於原岩攀岩館中的活動照片或影像拍攝可作為原岩攀岩館的紀錄或宣傳活動之用,若不同意,會事先告知岩館工作人員。
     

未成年同意聲明

本人(未成年者)及法定代理人已詳閱並同意上述責任聲明內容,並願遵守原岩攀岩館之規定。

Safety Code & Statement of Personal Responsibility - T-UP Climbing-Wanhua

★ All visitors entering T-UP Climbing Gym for the first time are required to read theSafety Code & Rulescarefully and complete theLiability Waiver. Those who enter without climbing are also required to read and sign.

★ Those who enter the gym for the first time must go through [Bouldering Safety Landing] practice with a staff member. If staff determines participants are unable to correctly perform safety practices according to the guidance, for their own safety, the gym has the right to stop participants from climbing.
 

Admissions for Minors (Under age of 18)

  • Minors (under age of 18) must jointly with their parent(s) or legal guardian read, understand theSafety Code & Rulesand have their waiver filled out by their parent(s) or legal guardian.

  • Minors under the age of 18 must be accompanied by an adult guardian when entering the climbing gym. The accompanying guardian shall be jointly and severally liable.

  • When accompanying a child under the age of 12, the guardian should keep an eye on the child’s condition at all times, and personally ensure the safety of the child. Except the children’s area, please do not let children run and play in other areas of the gym.
     

Gym Rules】

  • No dangerous goods are allowed in the gym. Smoking and drinking alcoholic beverages are prohibited. No fires are permitted.

  • Climbing barefoot or being shirtless is not allowed. Please wear climbing shoes when climbing.

  • Food and drinks (including sports drinks) are only permitted in the dining area. Water can be consumed outside of the dining area. Do not drink water on the climbing mats.

  • Please carry out all trash when leaving. Trash cans are not provided in the gym.

  • Pets must be kept on leash in the designated area of the gym. If pets bark, frighten or disturb other climbers, the gym has the right to ask the owner to take the pet out of the gym.

  • Any member or guest who steals or intentionally damages any property of T-UP will be held financially responsible for the damage and subject to prosecution.

  • If you wish to conduct any commercial-related activities within the gym, including commercial video or photo, please ask and obtain consent from T-UP in advance.

  • Some areas might be closed temporarily for route setting, courses or events. Your understanding is appreciated for not entering the closed areas.

  • We recommend not to bring any valuables to the gym. Please ensure the safety of your own valuables. The gym is not responsible for any loss of property.

  • The gym provides public accident liability insurance. The amount of insurance is insured in accordance with government regulations. The admission fee does not include travel insurance and personal accident insurance. Please insure yourself if necessary.
     

Safety Rules】

  • Before climbing, please monitor your body condition. Choose the climbing route and difficulty according to your ability to avoid overloading your body. Properly warm-up and cool-down to reduce sports injuries.

  • Before climbing, make sure your route does not overlap with others already climbing on the wall. If you find that you are too close to others or the routes may overlap, for the safety of both parties, please stop climbing, return to the ground and exit the climbing mats.

  • Please give priority to climbers who are already on the wall. When someone is climbing, do not idle in their surrounding area (including wall and mats).

  • When passing through the climbing area, please make sure you are not in the climber or belayer’s surrounding area, and pass through as quickly as possible.

  • When in the climbing area, be aware of your surroundings. Sitting on the climbing mats is prohibited and it is recommended to keep a distance of at least 2 meters from the wall to ensure the safety of yourself and climbers.

  • There are climbing mats in the bouldering area, please use correct safety landing form and jump from a safe height. Before jumping, make sure there are no people or objects below you. Also pay attention to the safe fall range to avoid danger of falling off the edge of the mats

  • After landing on the mats, please evacuate the mats as soon as possible. Do not idle on the mats.

  • No personal belongings are allowed in the climbing area or on the mats.

  • In case you notice any loose rocks or holds, damaged climbing mats, or accidents, please notify the staff immediately.

  • If there are matters not covered in the regulations, the staff of the gym can make appropriate decisions according to the situation on the spot and revise the relevant measures.

  • If a person violates the safety rules and refuses to listen to the staff's dissuasion, his or her membership will be revoked and he or she will not be allowed to enter the T-UP Climbing Gym again.
     

Statement of Personal Responsibility

  1. I understand that rock climbing (including bouldering and rope climbing) is a sport that contains certain risks including serious injuries or even death. Signing this statement of responsibility means that I have read and understood the regulations related to rock climbing, such as climbing, safety landing, and belaying safety rules of T-UP Climbing Gym. In addition to being willing to abide by all relevant regulations, I also understand and accept to bear all risks, crises, possible personal injury, death, or property loss, and am willing to take full responsibility for my own safety and behavior.

  2. I fully recognize that bouldering/climbing is an extreme sport which requires high level of physical exertion. I have considered my health and physical condition and voluntarily enter this facility. I do not hide any existing or past mental or physical health issues.

  3. I agree to photos taken by T-UP Climbing can be used for promotion for T-UP Climbing in any way. If I do not agree, I will let T-UP Climbing know in advance.
     

Legal Representative's Agreement

I (a minor) and my legal representative have carefully read and agreed to the above responsibility statement and are willing to abide by the regulations of T-UP Climbing Gym.

原岩攀岩館(以下簡稱本岩館)為蒐集、處理及利用您的個人資料,茲依據個人資料保護法第 8 條第 1 項規定,向您告知下列事項:

若欲使用本場地,請仔細閱讀「安全守則及使用規則」以及「個人責任聲明」並填寫同意書。首先,請您先閱讀以下個人資料保護法相關事項。當您填寫資料並送出後,本岩館將視為您已同意提供您的個人資料供本岩館依特定目的必要範圍內處理及利用;依據個人資料保護法等相關規定,本岩館有義務告知以下事項,請您務必詳閱,確認同意後再勾選並填寫個人資料。

一、蒐集目的:

(一) 消費者、客戶管理及服務(代號 090)

二、蒐集之個人資料類別:

本岩館蒐集您的個人資料類別,依法務部「個人資料保護法之特定目的及個人資料之類別」之分類計有(代號C001):個人資料識別類:辨識個人之姓名、性別、聯絡方式。

三、個人資料利用之期間、地區、對象及方式:

(一) 期間
於主管機關許可業務經營之存續期間內,符合下列要件之一者:
1、個人資料蒐集之特定目的存續期間。
2、本公司因執行業務所必須之保存期間。
(二) 地區
所有蒐集之個人資料將於中華民國領域範圍內使用。
(三) 對象
原岩攀岩館。
(四) 方式:
用於確認消費者同意遵守場館安全規定,並充分了解攀岩運動之風險。

四、您就本岩館保有您個人資料,申請者之身分經確認後,得向本岩館要求行使下列權利:

(一) 查詢、請求閱覽或請求製給複製本,而本公司依法得酌收必要成本費用。
(二) 請求補充或更正,惟依法您應為適當之釋明。
(三) 請求停止蒐集、處理或利用及請求刪除,惟依法本岩館因執行業務所必須者,得不依您請求為之。
您欲行使上述提及之相關權利時,本岩館櫃台能受理您的請求,您可以在我們的官方網站上找到聯繫我們的電話號碼及
分店地址。

五、您拒不提供個人資料所致權益之影響:

您拒不提供個人資料,基於安全考量,本岩館將無法讓您進入岩館進行攀岩活動及提供其他相關服務。

六、未成年消費者入場說明:

若入場者未滿 18 歲,請由法定代理人陪同閱讀以上說明,並填寫資訊與簽署同意書 。

All visitors to T-UP Climbing (“T-UP”) for the first time are required to read the 【Safety Code & Rules】carefully and complete the 【 Liability Waiver 】. Information Collection

Upon admission to T-UP Climbing, we ask you to provide us with certain personally identifiable information that can be used to contact or identify you. Personally identifiable information may include, but is not limited to your name (“Personal Information).

Disclosure and Use of Personal Information

T-UP will not disclose, share, or sell individual personal information with anyone without your prior consent unless ordered by a court of law. Information submitted to us is only available to designated personnel. T-UP will only use the information to contact your for event or courses purpose, or when in a emergency.

Disagree to the Collection of Personal Information

T-UP may not be able to provide you relevant services such as admission to our facilities or contact you in an emergency due to the lack of your personal information. Also, you may lose certain rights if you provide incorrect, untrue, outdated or incomplete personal information.
